I worked for 3 years in a busy hospital based OP neuro clinic. Through the years, I began to notice people with chronic conditions(think SCI over a year old, a year or more post stroke, chronic MS flare) be admitted, discharged, readmitted, redischarged etc. from our clinic. Plans were never much different and people saw minimal progress, hence the need to come back, over and over and over.

I began to realize that our current system is not set up to manage people with chronic neurological conditions and wonder if there was a different way to do neuro rehab.
